FEES

Sessions are generally 55-60 minutes in length. My fee for this length of session is $120. For private pay (no billing required) the fee is discounted slightly to $100. Insurance benefits vary. If you are planning to use insurance benefits, please contact me with your information so we can check on your benefits. The full private pay fee or the co-pay and/or co-insurance percentage (if using insurance) is due in cash or check at the time of the session. 
DIRECTIONS TO MY OFFICE

409 E. Sumach Street is an office building in a residential neighborhood near the Rose Street Safeway and the Elks Club.

Heading East or West on Highway 12. Take the 2nd Avenue exit. At Rose Street, take a left. Two blocks after the Safeway, take a Left on Tukannon Street, just past the Elks Club. Take your next Right on Sumach Street; number 409 is the second house on your left. The number and a sign board is by the front door.  Park on the street in front of the house or in the small parking lot at the rear of the house (requires going down the alley that runs parallel to Sumach). Entering the house through either the front or rear door, you will pass an office and then come to the open waiting room area. Please make yourself comfortable there. APPOINTMENTS

I am available for appointments weekdays, generally in the afternoon and evening. The next appointment time will often be scheduled at the conclusion of the session. For missed appointments without prior notification (“no show”), the full private pay fee will be charged. For cancellations after 9:00am on the day of a scheduled appointment, one half of the private pay fee will be charged. Please be advised that insurance does not pay for either late cancellations or missed sessions, so you will be responsible for the charge. Also please know that the late cancellation fee does apply even in the event of family emergency, sudden illness, or unexpected problem with childcare. I encourage you to reschedule if you anticipate a likely problem rather than hope for the best and risk incurring the late cancel fee. I realize these charges can be burdensome. They are, however, an unpleasant necessity for sustaining a by-the-hour business.  (to top of page)



INSURANCE

If you have insurance benefits that you would like to use, I am glad to assist you in determining what those benefits are and in billing the insurance company for you. I am currently in network for REGENCE BLUE SHIELD (including UNIFORM and FEP), PREMERA BLUE CROSS, MEDICARE, and MODA. I am out-of-network for other insurance companies (including Aetna, Kaiser, First Choice Health Network, Group Health, and United Healthcare). Please note that insurers do not pay for missed sessions or late cancellations. Also, in the event that the insurance does not pay as expected, you will be responsible for the balance.
